How does this work?

Whether you click or call, Essen Care Coordinators will get right back to you, begin monitoring your care right away, and ultimately connect you with the healthcare professional that’s right for you.

  • ONLINE FORM / CALL
    Contact us via online form, Patient Care Center or your assigned Care Coordinator. If they can handle your issue, they will. Otherwise, they will schedule a ”Virtual Visit” so you can directly connect with a provider from your home.
  • VIRTUAL VISIT
    Your provider will manage your problem remotely, order tests, medicines, or make the appropriate referrals, and then schedule a Virtual Visit follow up to check in on your condition, within a day or two.
  • IN-PERSON
    If the provider feels you must have in-person evaluation and care, they will schedule an appointment at one of Essen’s offices or clinics, or schedule a housecall, with a primary provider or specialist.
  • OUTSIDE REFERRAL
    If your provider feels you need to see a provider or specialist not on our staff, or if your situation is an emergency, they will ensure you are referred to the most appropriate care, as quickly as needed.

Care Coordination

Our Care Coordinators can keep in touch with you to help, even when you are not scheduled for an appointment.

As part of this commitment, we can connect you with a “Care Coordinator,” who will spend some time getting to know you and help you identify and connect with all the services you need. Ask your provider or the Patient Care Center about your eligibility for Care Coordination at Essen.
