The mission of the Essen Health Care Nursing Home Division is to create an integrated system of care dedicated to delivering high quality, accessible and cost-effective medical services for Nursing Home residents.

Essen Health Care has an extensive network of over 300 healthcare providers who deliver medical care to residents in over 60 Skilled Nursing Facilities.

Model of Care
Our model of care is predicated on an evidence-based predicative analysis with integrated geriatric care by full-time Clinicians and Specialists. We aim to decrease avoidable hospital admissions and readmissions rates while providing continuity of care.
Essen Health Care is a collaborative team delivering optimal care for all of our patients in the Hospital, Community, Assisted living or Urgent Care settings.
